Camp Forms

Thanks for registering your camper for our 2013 Summer Camp! Please fill out these forms to provide us with necessary information and legal release for your camper.

Camper Information Packet:

This includes emergency contact information, physician and insurance information, health history, medications and treatment authorization, specific activity permissions, and a media release.

Physical Exam Form:

This MUST be completed by the Camper's physician prior to camp.

Grady Spruce Release Form:

This MUST be completed for your camper to participate at our host camp site.

Note: You don't have to submit this information at the same time as registration, but it is strongly encouraged.
